I began taking care of hospice patients back in 2015 after moving to San Diego and working in a hospital setting for a while. We also have a home health aide, a chaplain, a social worker, a patient intake coordinator and a medical director. Luckily, we have been able to stay fully staffed.

By 2007, the Washington State Agency Medical Directors Group (AMDG), which included several eventual PROP members (23), published an “Interagency Guideline on Opioid Dosing for Chronic Non-Cancer Pain (26) that introduced the concept of hard dosing thresholds for opioid analgesics, which was later incorporated into the 2016 Guideline (2, 3).

We started having palliative care IDGs, where we would have our medical director sit in on case management and case conferences to understand “should we have actually coded this differently than we did?” I think we’re coming a long way. I looked for a palliative care program that served pediatrics in my area.

I think the big one for me, it was in 2015. 2015, 2016. The MVP for them is care navigator, medical director, or a clinician who could be advanced care practice PA or a physician who have one of three factor. What we didn’t have was the payment model to deliver the comprehensive dementia care. Malaz: Yeah.

And I became the medical director for that program, and so I was watching and learning as the hospital was taken over by AIDS patients and the whole city was consumed by this epidemic. And so I decided to look at medical aid in dying. And this was in 2015 when I started to think about this project.
