Strong staff education and reimbursement are among the keys to successfully integrating social determinants of health within a palliative care program. Hospice teams typically include physicians, nurses, socialworkers, spiritual counselors, pharmacists, and other trained staff who specialize in managing pain and symptoms while offering emotional and spiritual support to both patient and family during the difficult time when a cure is no longer an option.
